Job summary An exciting opportunity has arisen for a motivated Practice Nurse to join our dynamic team at Cardigan Health Centre for 27 hours per week. You will be working within a supportive, friendly team across nurses, doctors, reception and admin staff. We have a Practice population of circa 10,300 patients and we work collaboratively with MIU/SDUC, physiotherapy and community nurses, who are all based within the Integrated Care Centre. Main duties of the job We are looking for a general Practice Nurse who will also cover treatment room duties. You will be responsible for ensuring the delivery of safe and effective nursing care to the whole Practice population, clinically delivering evidence-based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients. You will work collaboratively with the whole Practice team to meet the needs of patients, whilst supporting the delivery of procedures. Experience in Diabetes care and monitoring and in a General Practice environment is desirable, but full training will be given to the right candidate. You will be supported by the GP lead for diabetes and the community diabetes nurses. We require a degree of flexibility as occasional overtime will be required to cover annual leave or sickness absence. About us Our team is made up of 4 GP Partners, 2 Salaried GPs, 3 ANPs, 4 Practice Nurses, 1 HCA and 1 Phlebotomist, along with a large Reception and Admin team. We are a training Practice and we currently have 3 trainee GPs working with us. We will also be mentoring student Nurses.
