We are looking for a health professional who has a passion for making a difference in the lives of people with LD and Autism. The NW London ICB has an established LeDeR team to cover the boroughs of Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow and the Royal Borough of Kensington and Chelsea and Westminster. The Team is responsible for the smooth running of the LeDeR process; ensuring all deaths with Learning Disabilities and Autism are reviewed in a timely and effective way., We are looking for an experienced health professional who will support, along with the Local Area Coordinator, all LeDeR programme activity across NW London. We are looking for someone with who is able to undertake LeDeR reviews to a high standard, drawing out good practice and recognising where improvements need to be made. We need someone who has good communication skills to support working across the different agencies involved with the LeDeR process. Good communication skills are essential as you will be expected to liaise and work with families/guardians of learning disability patients to ensure their involvement throughout the process. We need someone who can analyse data and identify trends and themes coming from the reviews and make recommendations for areas of improvement and disseminate the learning across a wide audience. The LeDeR service sits in the Chief Nursing Directorate of the ICB, and it works very closely with the NWL Safeguarding Team, the Child Death Review Team and the services across the NWL Integrated Care System. There will be close relationships with the Safeguarding Partnership Boards across NW London to ensure learning is disseminated across a multiagency setting. There are strong working relationships across the Chief Nursing Directorate which enables staff to be supported and developed and feel part of a wider team., Working for us, you can help ensure that our residents and patients have access to the right services when they need them and assist them take greater care of their own health to ensure they live longer, healthier lives.
