An exciting opportunity has arisen to join a busy and passionate CMHT within the centre of Swansea as a CPN. The role will involve working as part of a multidisciplinary team, undertaking the role of Care Coordinator in delivering high-quality community mental health services under the framework of the Mental Health Measure (2010) Wales, while maintaining a Recovery ethos at the forefront of nursing care.
Main duties of the job
To assess, implement, and evaluate care and treatment plans for each individual on the caseload, in collaboration with service users and carers. The role requires autonomous working under indirect supervision of a Band 7 registered nurse/team manager, in conjunction with other health colleagues, to support those under the care of EIP.

Detailed job description and main responsibilities
The successful candidate will work autonomously and as part of a team, conducting mental health assessments and serving as a point of contact for existing service users in the absence of their care coordinator. The role may also involve facilitating clinics within the CMHT, performing physical health checks, or medication administration in line with local policy.
Person specification
Qualifications
* Registered Mental Health Nurse
* Evidence of post-registration study (e.g., postgraduate diploma) or equivalent registration experience
Knowledge
* Clinical knowledge base, including current legislation such as the Mental Health Act 1983, Mental Health (Wales) Measure 2010, Mental Capacity Act, and Deprivation of Liberty safeguards
* Understanding of current care and treatment for adults with mental health needs
* Knowledge of the Clinical Governance agenda
Experience
* Experience working in multidisciplinary and multi-agency settings
* Experience in risk assessment and management
* Experience at Band 5 or above in a related clinical environment
* Leadership experience within a team
Skills
* Excellent communication and interpersonal skills
* Ability to travel daily between sites and patient homes efficiently
