About Darwin Medical Practice:
Darwin Medical Practice provides services from Greenwood Health Centre, Lichfield Road, Burntwood, St Chads Health Centre, Dimbles Lane in Lichfield and Chasetown Medical Centre, 29-31 High Street, Chasetown, Burntwood. Our aim is to deliver a high quality service which will benefit our patients. With a large team working together we will maintain and improve the quality of care with the sharing of expertise and skills, enabling patients to have more choice. We have a coordinated approach, developing our teams to support patients with long term and complex conditions.
We value our relationship with our patients and have an active Patient Participation Group to help us deliver the best service possible.

Job Purpose:
To provide a high-quality care service based on the needs of the practice population which includes treatment, screening, preventative care and patient education.
Principal Accountabilities:
1. Manage programmes of care for patients with chronic diseases.
2. Administer adult and child immunisations and vaccinations.
3. Undertake procedures including venepuncture, BP management, and wound management.
4. Ensure clinical practice is safe and effective.
5. Act as chaperone when required.
6. Undertake health promotion to help educate patients.
7. Offer comprehensive travel health advice.
8. Ensure accurate and legible notes of all consultations and treatments.
9. Participate in practice audit and research projects.
10. Maintain own annual registration with the NMC.
11. Promote a professional approach at all times.
12. Attend and participate in staff and practice meetings.
13. Any other reasonable duties that may be required.
The Practice is open from 8.00am until 6.30pm, with regular extended hours surgeries. The contents of this job description and person specification will be reviewed on an annual basis in line with the Practice training and development review policy.
