Job summary The Hyndburn and Ribble Valley team are a friendly, dedicated, supportive and flexible team, who strive to provide the best care possible with the resources we have within and local area. We are looking for individuals with a passion for community mental health, who will help us deliver this to our Service Users.We are looking for registered mental health practitioners (qualified social workers, mental health nurses or occupational therapists) to be part of our team. Ideally, you will have 18 - 24 months post qualification experience. You need to be able to work as part of a multidisciplinary team and autonomously and have experience of assessing, planning and implementing and reviewing care needs, risk assessment and management, and the ability to explore issues relating to the impact of health and social needs on mental health and well-being.Hyndburn and Ribble Valley CMHT covers some of the Hyndburn area and Ribble Valley areas and you will be expected to work within those area's, visiting service users, and at your base at The Mount, Accrington. Main duties of the job 1. To undertake mental health assessments including risk assessments within a multi-disciplinary assessment team. Working closely with the Consultant Psychiatrist, thus ensuring that there is a multidisciplinary team approach 2. To be proficient in referring to other clinicians within the service or referring to other services/agencies where necessary. 3. To provide a fast, responsive service to assess individuals needs for access into mental health services 4. Providing a communication structure between GPs, IAPT and secondary MH services in LCFT, non-statutory service to ensure the highest possible standards of patient care 5.
