A 37.5hr District Nurse/Community Sister/Charge Nurse post is now available in Adur Community PCN Team due to successful promotion.

There have been many changes in how patients receive their health care and this is ever increasingly provided in the community setting and it is definitely the place to gain skills and development in Case Management, Physical Assessment, Prescribing and managing Long Term Conditions and many Clinical Skills.
Development of your leadership skills are essential and we will provide the necessary training and support to achieve this.
Supporting Junior and new staff is very much part of the role and you need to have Practice Assessor/Supervisor and experience is essential to support the different levels of students.

Main duties of the job
The role includes using your clinical skills for assessment, diagnosis, treatment and/or appropriate referral in collaboration with other agencies to ensure care coordination, and includes being named as Key Link for individual practice(s). This also includes attending GSF, MDT's, Individual Patient Reviews, Hospital Discharge Planning and CHC Assessment and Reviews.
As well as the GP's and Social Services, you will also be expected to develop and maintain relationships with Nurse Specialists within SCFT, our partners in Palliative Care, Sussex Partnership NHS Foundation Trust and the voluntary sector in our community.
