Thisnew and exciting post supports various educational and research activitiesrelating to Heart Failure projects. We recently secured funding to supportthe Yorkshire & Humber Heart Failure Academy. This project is deliveredwith the support of Health Innovation Yorkshire and Humber. You will have theopportunity to develop your educational and research skills among wellestablished, award winning, multidisciplinary clinicians and researchers.
Thispost is a fixed term contract until 31st March 2025 and part time role of 7.5hours per week.

Therole involves supporting the clinical Heart Failure Multidisciplinary Teamleading the academy. You will be involved in developing, implementing, andoverseeing the educational content and programs for the Yorkshire and HumberHeart Failure Academy as agreed with the HF MDT. There will be opportunitiesto collect data and publish findings to evaluate and disseminate goodpractice. This position will ensure that all educational materials areclinically accurate, align with NHS best practices, and meet the learningneeds of healthcare professionals across the region.
Inthis role you will be expected to engage with healthcare professionals acrossYorkshire & Humber to promote the Academy and gather feedback oneducational needs. You will collaborate with other stake holders to developnon-promotional educational content.
Thepost holder will have the opportunity to support other Heart Failure projectswithin the unit.

PrincipalDuties & Areas of Responsibility
Clinical
Provide a highly specialisedclinical pharmacy service to patients and the multidisciplinary team within agiven clinical speciality.
Supports the Trust's PrescribingStandards and challenges practice where prescribers have not adhered to theMedicines Code.
Demonstrate the ability to providesafe, clinically effective and cost-effective use of medicines in accordancewith Trust policy, standard operating procedures and medicine legislation.This will include individual prescription review to optimise therapy,advising on dosage and side effects and on safe administration of medicines.The post holder is expected to determine how the outcomes are best achieve inspecific circumstances.
Pharmacists who are registered asprescribers will, with the agreement of the clinical director of thespeciality, prescribe within their level of competence, to support patientcare.
Plan, manage, monitor and reviewindividual patient care plans.
Participate in consultant wardrounds and multi-disciplinary team meetings.
Manage difficult and ambiguousproblems, often when information is limited.
Undertake risk management,including investigating clinical incidents in the clinical area and ensuringcompliance with medicine legislation.
The post holder will be expected toidentify, where necessary, areas that require service changes and develop astrategy to carry out the change through consultation with senior colleagueswithin and outside the cardiology.
Manage the introduction of newdrugs, including unlicensed drugs, into the clinical speciality.
Manage the clinical pharmacistinput into clinical trials.
Develop and update policies for thesafe, effective and economic use of medicines through discussion with medicaland nursing staff.
Communication with Primary Careabout medicines policies or guidelines specific for their area of practice.
Lead on specific aspects of theMedicines Safety agenda and provide expert opinion, both within and outsidecardiology within the Trust to ensure the safe and effective delivery of drugtherapy.
Take an active role in review ofincidents and sharing learning from these in cardiology and across the Trust.
Project manage the development andmaintenance of specific Trustwide polices/guidelines and other action plansthat are necessary for effective Medicines Safety eg. Trust InjectableMedicines Code.
Play a key role in theinvestigation of medicine related incidents and agreement of action plans.
To work across the MedicinesAdvisory Service team as required.
Assist the Lead Pharmacist -Innovative CaReMe Medicines Optimisation Services in developing anddelivering medicines-risk related education and training to healthcareprofessionals.
Management
Monitor and interpret expenditureon medicines within own clinical speciality. Provide the Directorate withregular reports to assist in the management of the drug budget
a. Attend the monthly ClinicalSpeciality Management meetings to advise on medicine use and financialimplications
- Horizon scan for new drugs andadvise the clinical speciality meetings on the financial implications
b. Co-ordinate Medicines Use SavingsProjects
c. Review prescribing data fromout-patient clinics
Contribution to financialinitiatives
Write or respond to business casesfor service development to secure additional funding.
Responsible for day to daysupervision / management of specialist clinical pharmacists in clinicalspeciality (some staff)
Researchand Education
Participate in research anddevelopment within clinical speciality
Undertakes own research/auditwithin clinical speciality
Supervise, teach, and assess staffcompleting training modules within the clinical area. This input varies,mostly based on the experience of the post holder, and will typicallyinclude:
- Acting as a trainer to pre-registrationand junior pharmacists in their area of specialist practice
- Being a named tutor for apre-registration and/or diploma pharmacist
- Lecturing on clinical issuesrelevant to the specialty, as part of the pre-registration training program,the clinical diploma, and any other in service training.
The post holder is also expected toparticipate in the training of other clinical professionals within theirspecialist area as requested.
Develop the role of a specialistpharmacist through national links.

Person Specification
Experience
* Prior experience in co-ordination and delivery of educational events, including webinars, workshops, and podcasts
* Clinical pharmacy knowledge in Heart Failure
* Ability to demonstrate foundation level competencies in all 6 domains of the Advanced to Consultant Level Framework (ACLF)
* Substantial post registration experience as a practising clinical pharmacist.
* Prior experience in developing educational materials / programmes
* Development of educational programmes
* Proven experience in medical education or clinical training roles
* Working across different specialties and healthcare systems
Qualifications
* Masters Qualification in Pharmacy (or equivalent)
* Registrant of the General Pharmaceutical Council
* Postgraduate diploma in clinical pharmacy or equivalent
* Non-medical prescriber
