We have an exciting opportunity for a Primary Care Nurse Manager to join our established team at Carlton Group Practice. We are looking for an enthusiastic, committed, dynamic and forward thinking individual to work collaboratively across the multi-disciplinary clinical and management team to support the delivery of outstanding care to the patient population, including acute and chronic condition management.
The successful candidate will have the ability to manage, motivate, train and support the Nursing Team consisting of 4 Practice Nurses, 2 HCAs and a Phlebotomist.The successful applicant should be an experienced Nurse with proven ability of the above.
They will be forward thinking and have the vision and ability to be able to drive change and enhance service development to provide optimal primary care services for the benefit of the patient population.
This role is based between our two surgeries, Carlton Street Surgery and King Street Surgery. However, we have received NHSE approval for a new build surgery based on the Outwoods site opposite Queen's Hospital. The successful candidate will assist the Senior Leadership Team in the smooth transition to the new site evaluating current and potential new services.
This is a full-time role, however part-time hours will be considered.
Main duties of the job
The post holder will have responsibility for providing managerial and professional leadership to the nursing and nursing support team. They will work closely with the Business and Operational Managers to deliver, plan and future proof services, leading the team to provide an exemplary clinical service whilst ensuring practice targets are maintained and achieved to optimal levels.Working as part of a multi-disciplinary team the post holder will be accountable for the delivery and standards to ensure safe, effective, person centered care.The post holder will have broad knowledge and experience of treating and working with patients and multi-disciplinary teams to maximise health outcomes within all Chronic Disease Areas.The post holder will be responsible for the Management of Infection Control across both sites and will be able to evidence relevant experience and training within this area.

To re-validate and renew Registered Nurse status as required by the Nursing and Midwifery Council (NMC) and ensure all members of the Nursing Team are revalidated and hold current registration with the Nursing and Midwifery Council (NMC) where required dependent upon their role.Support all members of the nursing team, providing guidance when necessary, conducting appraisals, training needs analyses and personal development plans.
