Job summary
The post holder will be a Registered Mental Health Nurse with a high level of expertise and experience in Children and Young People's (CYP) Continuing Care, responsible for managing the SCAMHs Children's Continuing Care Service.
The post holder will be responsible for the implementation of the Children's Continuing Care (CCC) process for all CYP's ensuring holistic assessments are undertaken to determine eligibility in accordance with the Welsh Government Guidance.
The post holder will have well-developed leadership qualities and will be required to draw upon their advanced specialist knowledge and offer direction, support and consultation to other professionals in relation to CCC.
The post holder will be the senior operational lead for CYP Continuing Care (Specialist CAMHS) and will work with the Childrens Continuing Care Lead (Physical Health) and Service Delivery Manager Paediatrics as well as the SCAMHs service manager in developing policy changes and implementation of relevant strategies.

The post holder will be required to promote the ethos of the service, which is to provide crisis assessment, intensive intervention including home treatment with the key aim to prevent admission or if admission is required to facilitate earlier discharge for children and young people.
Main duties of the job
The successful applicant will also :-
Be responsible for clinical interventions working specifically with CYP where there are identified concerns in relation to their emotional/mental health and present with additional complex needs considering the need for a responsive outreach approach.
Provide leadership from a mental health perspective to the multi-disciplinary team providing assessment and therapeutic intervention for a wide range of emotional and mental difficulties affecting young people and their families and carers.
Act as a care coordinator for a limited caseload of CYP with complex needs and ensure that the Care and Treatment Plan (CTP) forms the basis for the clinical intervention.
Be responsible for the coordination and delivery of training workshops for health, social care and education practitioners concerning the Children's Continuing Care process.
Work autonomously in collaboration with the S-CAMHS senior managers and the SDM Community Paediatrics, ensuring excellent working relationships, communication and working practices are further strengthened with Adult Mental Health, Learning Disabilities, Substance Misuse Services and partner agencies.

Essential
1. Registered Nurse (Mental Health)
2. Current NMC registration
3. Post graduate diploma qualification plus Masters' qualification or demonstrable equivalent experience of working with children and young people with complex health needs.
4. Ability to practice within the scope of the NMC Code of Professional Conduct.
5. Evidence of professional development & lifelong learning.
6. Knowledge of health and social care policy, Including the Children's Continuing Care Guidance and Social Services and Wellbeing Act.
Desirable
7. Educated to Master's Degree level or equivalent relevant experience.
8. Understanding of Budgets/Budgetary Management.
9. Training in any other evidence based psychological approaches ( DBT, Mindfulness based approaches, EMDR, CBT etc.
