Cwm Taf Morgannwg is looking for a forward-thinking and compassionate band 6 CPN.
This is an exciting opportunity for the right candidates to become part of a strong, well-established MDT with a clear service user focus and close working partnerships with other health professionals and the local authority.
The post holder will manage their own caseload of patients, have excellent communication skills, possess a positive attitude to working with people with complex mental health needs, whilst working effectively within the MDT.
The Rhondda CMHT are based at Pentre Municipal Buildings and the Taff Ely CMHT based at Dewi Sant Health Park.
The teams are supportive and inclusive, and opportunities for training and development are promoted within the role.
Responsibilities:
1. To deliver evidence-based holistic care to clients within the context of a multi-disciplinary, multi-agency community mental health service.
2. To hold a clinical caseload providing comprehensive assessment, care, and treatment to clients referred to the Community Mental Health Team in accordance with Part 2 of the Mental Health (Wales) Measure 2010.
3. Promote a positive working culture, working collaboratively with service users ensuring that their views and ideas are heard and equal.
4. Foster robust, open, and positive working relationships with all partners, in particular Local Authority and other mental health services and the 3rd sector, ensuring that all partners are treated with dignity and respect.
5. Proactively contribute to the continuing improvement and development of community mental health services, within an environment that welcomes new ideas and listens to everyone.
The post holder will be guided by broad occupational policies and will frequently work as a lone worker.
