We are looking for a dedicated Nurse (NHS band 6 equivalent) or AHP to join our community-based team. This role involves supporting the identification and management of individuals over 50 who have experienced a fragility fracture. Fragility fractures are often the first sign of osteoporosis and typically result from minor falls. They represent a significant healthcare cost, with predictions indicating a sharp increase in fracture numbers across Europe by 2030. The FLS aims to systematically identify and manage all eligible patients in the West Suffolk area to reduce their risk of subsequent fractures.
What is the role?
Main Duties:
* Conduct specialist bone health assessments and develop, implement and evaluate care plans.
* Provide expert advice on fracture prevention and osteoporosis to patients, families, and carers.
* Assess falls risks for patients and offer guidance on prevention and management strategies.
* Administer and manage six-monthly denosumab injections.
* Work autonomously, taking responsibility for your practice and being accountable for the care delivered.
* Support the clinical lead in developing and delivering high-quality, evidence-based services.
* Provide specialist osteoporosis education and training to various audiences.
* Contribute to the development of the fracture liaison service through audit, meetings, protocols and training.
Additional Information:
* The role involves significant driving to visit patients across the West Suffolk area.
* A full driving license and access to a vehicle are essential.
Contract type:
* Permanent full time
* Part time (minimum of 3 days a week) would be considered.
