Job summary Due to expansion of the Sefton Paediatric Continence team, there is an exciting opportunity for a Band 6 Paediatric Continence nurse to join the Integrated Childrens Community Nursing team (ICCNT). The post holder will have continuing responsibility for the management of a defined Child / Young Person Caseload and delivery of Specialist care for patients who have been referred to the Paediatric Continence Service across Sefton. They will be responsible for the continuity of care. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload. This will be done via a holistic assessment of care needs and the provision of clinical care for patients/clients with additional & complex needs. Practice autonomously and demonstrate evidence based clinical decision making. Work within guidelines to identify and manage risk, reporting identified risks to the Continence team Lead.
