As the Transfer of Care Team Manager, you will oversee the operational flow across both our North and South teams. You will ensure that discharge processes are managed efficiently and in line with timescales, while also leading initiatives such as Discharge to Assess. The key responsibilities include leading, managing, and supporting the North and South Transfer of Care Teams. Additionally, you will oversee discharge processes to ensure compliance with timescales and maintain a safe, efficient patient flow. You will manage and resolve any operational challenges related to discharge, ensuring continuity of care. The Transfer of Care Teams operate within specific office hours, but that does not mean we will not consider flexible working opportunities.
