Job overview
* To provide a responsive patient discharge support service completing and tracking referrals from receipt to patient discharge.
* To support the wards on the safe and timely discharge of patients from the acute settings, including attending whiteboards.
* To oversee the completion and compliance with Red2Green and Criteria to Reside initiatives from a ward perspective.
* To update the sit rep report for reporting to national database, ensuring that it is accurate and in real time.
* Liaise with ward staff regarding changes in patient’s clinical status affecting discharge and data recording.
* To support the Head of Site operations, Capacity and Discharge Planning, to perform all allocated administrative tasks required to meet the needs of the service.
* To undergo training as necessary to meet the needs of new developments within the service.
Main duties of the job
* Contribute to the department, ensuring that it operates to the highest standards, for discharge planning.
* Be responsible for ensuring that all duties are carried out in accordance with the correct procedures and policies.
* Ensure that E-Track information is correct and accurate.
* Support workloads within the department and cover for colleagues in their absence.
* Act as a key to external organisations regarding queries related to discharge planning and specific patient enquiries.
* Deal sensitively and effectively with telephone enquiries from colleagues, ensuring confidentiality is always maintained.
* Actively take part in team meetings to ensure the department is flexible in its approach to changing workloads.
* Act as a named link for the discharge team to ensure that the appropriate level of knowledge is acquired to support the administrative processes required.
* To liaise with Social Services, health providers and organisations providing streamlined patient discharges as required.

* NVQ III Business and Administration
Experience
* Previous administrative/secretarial experience
* Experience of using MS Office, Word, Excel, and Outlook
* Demonstrate an understanding of customer care
* Experience of working in the NHS, use of NHS systems and understanding of medical terminology
* Experience of working with Multi-Disciplinary Teams and/or attending meetings
Knowledge
