An exciting opportunity has arisen to join a new innovative team within the Serious Mental Illness (SMI) and frailty pathway in NE Essex for older adults over 70 years of age with frailty. We are looking for a compassionate, motivated and caring individual whom has experience in working with people with a functional illness with or without frailty. As part of the national agenda for transformation the service will focus upon admission avoidance to both general hospital and a mental health inpatient setting.
The service will be working collaboratively with other health care providers, third sector agencies, service users, carers, clinicians and other practitioners. Close working with the community teams in the local neighbourhood model will take place in line with the North East Essex Alliance.
A key part of this role will be providing comprehensive assessment, as well as having responsibility for a case load of individuals. The role will entail providing person centred and evidence based interventions working alongside a multi-disciplinary team. There will be a requirement of ensuring quality interventions and high standards of documentation. The successful candidate will be required to have knowledge and experience of physical health and how this can impact upon an individual’s mental health.
The post holder carries responsibility for the assessment of care needs and the development, implementation and evaluation of programmes of care for patients with SMI with frailty Patients will be referred by GP’s Colchester Hospital, Community Nursing Services and Internal EPUT Teams. As the integrated/SMI with Frailty Pathway develops it is expected that the roles will in future be aligned to the North East 6 Neighbourhood Hubs.
The SMI with Frailty Practitioner will provide short-term intervention either within in the person’s place of residence or Colchester Hospital linking the needs led care plan to place based services including community services, social care and third sector organisations e.g. Age Concern. They will be expected to work collaboratively with individuals and their carer’s/family delivering person centred care. Actively participate in the training and education of staff, families and carers as required. In addition, they will also be responsible for ensuring that an integrated service is provided and ensure that good inter-disciplinary working is maintained.

As part of the ongoing North East Essex Alliance transformation of services and in line with the NHS Long Term Plan, development of Essex Partnership University Trust (EPUT) services for dementia have been working over the past year in developing an integrated Dementia and Serious Mental Illness with Frailty Pathway. This new and exciting role is to join EPUT as a Band 6 senior community practitioner. .
working to develop an integrated approach to assessment and intervention for patients with SMI and Frailty. g with a SMI and with frailty This role will include supporting ESNEFT inpatient and frailty services to facilitate timely discharge, care planning and identifying appropriate community place based services. The development of a fully integrated/SMI with Frailty Service is currently ongoing and this role will develop alongside the wider integrated pathway expansion.
The post holder will develop working relationships with a range of professionals, services and organisations within the North East Essex area. This includes the North East Single Point of Access (NESPA), ESNEFT Frailty Team based at Colchester General Hospital, Community Nursing, Community Mental Health Teams, Social Care, Community based non-statutory services as well as local Care Homes. Through this earlier intervention there is an expectation that the role will provide effective and efficient short-term support/intervention whereby time spent in hospital can be lessened and effective discharge planning can help to prevent re-admission. The goal will be to work with partners providing expert and proactive clinical care and advice, improve the quality of the care experience and life post discharge.
