Adult Locality AS-SG Are you an experienced Social Worker with a passion for supporting adults in need? Our client's Adults team is seeking a qualified Adult Locality AS-SG professional to join their dedicated team in the North West. Key Responsibilities: Conducting Person-Centered Assessments: You'll be responsible for conducting detailed assessments and developing care plans that are tailored to the individual needs of adults in a variety of settings. Collaborating with Multidisciplinary Teams: Work closely with healthcare professionals and other team members to ensure integrated care and support for individuals. Providing Guidance and Support: Assist adults and their families in accessing the right services and resources to improve their quality of life. Monitoring and Adapting Care Plans: Regularly review and adjust care plans to ensure they meet the changing needs of individuals. Building Community Relationships: Engage with local communities to reduce isolation and promote social inclusion. Ensuring Compliance: Adhere to social care policies, legislation, and best practices while maintaining detailed, accurate records in line with data protection regulations. What We're Looking For: Qualifications: Degree in Social Work (or equivalent), registered with Social Work England (or relevant professional body). Experience: At least 2 years PQE, with proven experience in adult social care and locality-based roles. Skills & Competencies: Strong assessment skills, excellent communication, time management abilities, and a genuine desire to support vulnerable adults.
