Job summary This post holder will be responsible for running and developing the Wirral Enhanced Recovery Programme (ERP). The main duties are providing pre-operative education to patients in an assessment clinic, being a key contact for patients whilst on the Colorectal Unit and undertaking a telephone follow up clinic post discharge. The post holder will use an electronic database to record and capture details relating to the patients journey which will support research and training. The post holder will be responsible for the assessment, planning and delivery of nursing care for a named caseload of patients and includes nurse led discharges from the Colorectal Unit's Enhanced Recovery bays.The post holder will participate in the education, development and supervision of other staff members. Main duties of the job o Implement the Enhanced Recovery Programme (ERP) on the Colorectal Unit.o Take responsibility for the assessment, planning, delivery and evaluation of the nursing care for a named caseload of patients under the ERP.o Assess and educate patients in the colorectal pre-operative.o Provide education and advice to patients and their carers about the ERP.o Provide teaching to junior doctors regarding ERP principles and present case studies.o Conduct a telephone clinic for patients discharged under ERP and provide open access to the Colorectal Unit to support a reduction in re-admissions and improve patient experience.o Audit and review annually the Enhanced Recovery Programme and implement any improvements identified to improve the programme.o Support other staff members within the team and wider MDT to further their own education and development in colorectal surgery and ERP.o Attend the daily consultant-led ward round with the aim to input specifically to the ERP patients.o Provide daily clinical updates on the ERP patients to support safe and effective discharge.o Maintain regular contacts with patients' family and/or carers during their admission.
