Planned community nursing is a busy, forward thinking provision, delivering nursing interventions to patients in their own homes. As a key member of the directorate leadership team, you will have an integral and influential role in managing the day to day operations of the community nursing service. If you are a strong leader with experience of managing teams and resource to improve patient outcomes, we would love to hear from you. You will need to have a proven track record of accomplishment in service improvement and innovative practice as well as the ability to support colleagues to deliver the best services possible. Main duties of the job The Service Manager is responsible for ensuring the day-to-day delivery of designated clinical services and ensuring that those teams are performing in terms of patient outcomes, metrics, measures and targets. They are also accountable for making sure that the Trust's strategy is executed locally and that standards of safety are maintainedat all times.The post holder will be responsible for ensuring the delivery of high-quality patient care that is consistent with the values and strategic vision of the organisation.
