You will need you will receive a comprehensive induction, and be supported by your peers to understand assessments and interventions with adults in our community. You will work with adults to assess their needs under the Care Act 2014, establishing positive relationships by using Signs of Safety (SOS) tools to understand the adults needs, formulate solutions and review progress you will need good to be a good listener, have sound communication skills and an understanding of the local community. You will need to have good computer skills to record your assessments, email and assessments. You will need to have an understanding of People at the Heart of Care Suffolk County Councils strategy to support the adults voice, choice and control and promote greater independence you will need to able to form positive relationships with other professionals, voluntary, community, faith and social enterprises to support positive outcomes for adults we work with. The teams The Flexible Care and Support Team consists of Occupational Therapists, Social Workers, and Independence and Well-Being Practitioners. They work with local providers, Continuing Health Care professionals, CYPS for young people moving into adulthood, and Older Adults with Mental Health issues. They are a hardworking, supportive, and friendly team with a wealth of knowledge and experience to share with a new Independence and Well-Being Practitioner. Waveney is a great place to work, with system-wide support, training, and development workshops. South Waveney Team consists of Occupational Therapists, Social Workers, and Independence and Well-Being Practitioners. They work with adults in the Lowestoft and Beccles area, conducting Care Act (2014) assessments and reviewing the adults' care and support needs. They work with local resources, health colleagues, and care providers to ensure the needs of the adult are met. Staff work with adults to deliver quality care solutions to promote greater independence, choice, and control whilst mitigating risk and ensuring sustainable care. They are a hardworking, supportive, and friendly team with a wealth of knowledge and experience to share with a new Independence and Well-Being Practitioner.
