Clinical Specialist - Nurse Practice Based

An exciting opportunity has arisen for a Band 7 Clinical Specialist - Nurse to work into Greater Middlesbrough PCN to support the needs of the local population living with mental health difficulties. This post supports people in primary care settings and works in a collaborative and multi-agency way to meet the needs of patients with a range of mental health needs.
The post will sit within the GP practices and will offer assessment, diagnosis, treatment, and review for both working-age adults and older people. The role will provide specialist mental health clinical care and networking expertise, ensuring each person receives the best possible care in the right place at the right time by the right supporting service.
Candidates should have extensive clinical knowledge and experience across a range of mental health settings. Candidates will need to hold or be willing to work towards a prescribing qualification and be able to offer diagnosis and treatment plans across all mental health conditions and presentations. Experience in leading meetings, presentation skills, and offering training and education to professionals across a range of specialties will be essential.
Main duties of the job
1. Work with patients who have a severe mental illness and/or complex emotional needs who do not meet the criteria for secondary care services and are too complex for IAPT services to support shared decision-making about self-management; facilitate onward access to treatment services; and provide brief evidence-based psychological interventions, where qualified to do so.
2. Work closely with other PCN-based roles to address wider patient needs, e.g., PCN clinical pharmacists for medication reviews, and social prescribing link workers for access to community-based support.
3. Operate without the need for formal referral from GPs, including accepting some direct bookings where appropriate.
4. Provide a consultation, advice, triage, and liaison function, supported by the local community mental health provider through robust clinical governance structures to maintain quality and safety, including supervision where appropriate.
5. Provide clinical leadership to other ARRS mental health practitioner roles, such as Mental Health and Wellbeing Coaches.
About us
You will be employed by TEWV but will be 100% deployed into the PCN. Support will be given to you by both the PCN and other staff that work in the Tees Valley PCN Service within TEWV. Other members of the team include an Associate Nurse Consultant, Team Manager, Advanced Mental Health Practitioners, Senior Mental Health Practitioner, and Mental Health and Wellbeing Coaches.

Person Specification
Experience
* Significant experience working with patients with complex needs in the specialist field.
* Significant experience working with patients who display risk behaviours.
* Clinical risk assessment and management and its application in practice.
* Working collaboratively with service users, their families, and carers.
* Working in a multi-disciplinary team.
* Quality improvement activities.
Qualifications
* Current Professional Registration with Nursing and Midwifery Council - RMN / RNLD
* Recognised Practice Assessor/Practice Supervisor or currently in progress of achieving.
* Trained in Clinical Supervision and providing Clinical Supervision or in the process of achieving.
* Postgraduate diploma in a subject relevant to the area or be willing to work towards and achieve within 2 years.
* Within the first year of being in post, hold a recognised qualification in a governed therapy/advanced clinical qualification e.g., Intermediate Systemic Family Therapy/Post Grad diploma in CBT/British Isles Dialectical Behaviour Therapy/NMP (Independent Prescriber).
* Have equivalent clinical experience to that of a Post Grad Diploma/master's level such as working as a band 6 (or above) in a relevant clinical area with significant experience, together with evidence of relevant CPD.
Knowledge
* Extensive knowledge of current issues relating to health needs of patients in the field of practice, regionally and nationally.
* Understanding and working knowledge of the legal framework and statutory obligations under the Mental Health Act, Mental Capacity Act.
* Detailed understanding of Safeguarding and its application in practice.
* Clinical Risk Assessment and Management and its application in practice.
* Clinical Governance and its application in practice.
* Working knowledge of clinical medicines management.
* Understanding of psychological models of care and treatment (depending on specialist area).
