Job summary The post holder will work in partnership with our Hospital at Home, consultants in older people's medicine, GP with specialist interest, pharmacists, nurses, therapists and other members of the multidisciplinary team delivering HospitalHome to older frail people across Hambleton and Richmondshire areas.The service aims to support frailty crises in the community and prevent admissions where appropriate. The HospitalHome team takes referrals from GPs, paramedics and other health care practitioners. The post holder would become part of the existing team and work in partnership with the wider members of the team. The majority of assessments are done in the patient's own home. The team consists of a lead consultant, OPM consultant, GP specialist interest and a team of ACPs, nurse, therapists, and pharmacy support. The team are integrated with the Urgent Community Response (UCR) team, The team meets every morning for "board round' and allocates face to face or telephone reviews by the most appropriate member of the team. We have a no wrong door approach to referrals and strive to provide excellent patient care. The role will have an education and mentoring role for everybody within the Richmondshire and Hambleton healthcare economy in the management of frailty. Although the senior clinical model is working towards a 7-day service. The post holder will be required to cover Tuesday or Friday's at this time. Main duties of the job The core functions of this post are: To provide clinical care in the form of comprehensive geriatric assessment to frail patients with a view to providing care at home where this is possible, including NEWS2 scoring, Rockwood scoring, 4AT scores and other formalised assessments. To understand the major frailty syndromes and their management To understand, communicate and hold clinical risk for patients who choose to stay at home To communicate with patients and their families the working diagnosis, options for treatment, management plan and any results that become available. To write and check discharge summaries for patients being discharged. To undertake mental capacity assessments where this is required To undertake venepuncture, obtain ECGs, bladder scanning, and rectal examination, and enema delivery where this is necessary. To support other members of the multidisciplinary team in their provision of clinical care to patients. To work in partnership with all other members of the HH/UCR senior leadership team. To work in partnership with the other members of the multidisciplinary team Facilitate and support early discharge for patients admitted to wards where possible. Provide senior clinical decision making to the team.
