Job summary Working within the Sexual Health setting with medical, allied health professional and nursing staff, alongside the current Substance Misuse Practitioner to provide holistic care to service users around ChemSex. The post holder will support service users from the point of entry into the service and through their treatment/recovery journey; provide screening, assessment, and recovery planning and onward referral; reduce drug and alcohol related harm to service users and the wider community; work in partnership with the voluntary sector around recovery and ongoing support. Main duties of the job Work as a part of a team to oversee assigned groups of service users, providing: screening; asset mapping; comprehensive assessments; recovery planning (using motivational interviewing techniques) and review of care. Co-ordinate and deliver a tailor-made package of care Carry out comprehensive service user assessments, including asset mapping of community resources and develop individual recovery plans that clearly identify how achieving each goal will enable progression in addressing substance use, sexual health and re-integrating into the community. Support interventions focused on behaviour change, motivation, confidence and building strengths. Ensure resources for interventions are available and stocks maintained. Work in partnership with colleagues and other services to ensure the wider needs of clients are addressed including health, mental health, SARC etc. Ensure the timely and accurate collection, recording and reporting of specific data and discussion with the MDT. Carry out risk assessment and management. Triage assessment and onward referral to a range of treatment/community support agencies.
