Reports To: VP, Care Management & Pharmacy Services
Schedule: Full-time Days. M-F
Salary Range: $58,000 - $79,500 DOE.
GRIPA is seeking a Social Work Care Manager to engage patients and collaborate with colleagues to deliver high quality, innovative, cost-efficient and supportive health care in our community.
Job Objectives:
The Social Work Care Manager will provide direct patient support both in-person and remote. They will be responsible for completing assessments, developing care plans, implementing interventions, and linking patients and families to community resources. The Social Work Care Manager will act as part of an interdisciplinary care team.
Job Responsibilities:
1. Work in settings across the care continuum including hospitals, provider practices and patient’s home and/or community settings
2. Complete assessments that address social determinants of health
3. Develop a care plan and implement interventions
4. Provide psychosocial support
5. Provide timely documentation of interventions with, or on behalf of patients
6. Actively participate in on-line and in-person educational opportunities
7. Monitor and measure patient progress
Required Experience:
1. MSW or Masters in a related field or BSW with 5 (five) years of medical and/or community based Social Work/Care Management experience
2. Knowledge of local community resources
3. Excellent communication and interpersonal skills
4. Ability to navigate complex medical and social systems
5. Exceptional documentation and planning skills
6. A collaborative and flexible spirit
7. Strong computer skills (competence in Microsoft Office, particularly Word, basic Excel, Outlook, Teams, and familiarity with electronic medical records)
Preferred Experience:
1. Home based service delivery experience
2. Bilingual skills
Additional Requirements:
1. This position requires local candidates only. Fully remote or out of area candidates will not be considered.
2. Remote work location / hybrid schedule will be considered
