This is a full-time position within the Integrated Discharge Team.
The post holder will support the Head of Department and their team to ensure that their workstream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery.
Manage the Team's business support function, including developing and managing the team's business plan, progress and reporting risk and issue management.
Main duties of the job
The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders.
Participate in relevant internal and external working groups/projects, services, and initiatives to provide information and analytical advice and expertise.
Support the Discharge Liaison Officers by developing and implementing data collection systems that will provide accurate and timely data.
Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.
Liaise with other Managers to share best practice.
Manage and maintain a training programme for all staff groups and banding, both face to face as well as producing and updating online training.

Job responsibilities
This role will manage the Health Side Workforce, rotas, and level of cover to meet the needs of the service.
Report to Deputy Head of Service any staffing issues.
Develop and implement a content management system to ensure information is properly managed and best practice is shared across the team, Directorate, and the wider NHS organisation.
Take a lead in the coordination of training & development and recruitment activity across the Team.
Lead on training and development for all Bandings and Staff Groups across the Trust.
Oversight and management along with the Head of Service, of all aspects of the Team budgets.
The above is only an outline of the tasks, responsibilities, and outcomes required of the role. The job holder will carry out any other duties as a Discharge Planning Nurse that may reasonably be required by their line manager.

Education and qualifications
* RGN
* Ability to think and practice at first degree level
* Recognised teaching certificate e.g. ENB 998
* Experience in DC planning
* Experience in Management
Knowledge
* Expert knowledge of discharge and related contemporary issues.
* Knowledge and understanding of health care needs in a multicultural society.
* Understanding of the role of the discharge coordinator.
* Expert knowledge of national guidance and targets in relation to discharge.
* Recent experience within discharge services.
