We have an exciting opportunity for a Care Brokerage and Placement Coordinator to join us!
This is a full-time, permanent role with hybrid working, and a salary of £28,193 - £32,104 per annum.

The Role:
We are looking for a self-motivated person to join our Adult Social Care Brokerage Team.
Your role will involve:
* Commissioning and procuring services for various client groups.
* Experience of negotiation, particularly in relation to health and social care services.
* Working jointly with Care Managers, Social Workers and Service Providers.
* Setting up domiciliary Care Packages/Re-starts/Suspensions and ending care packages in addition to sourcing Nursing/Residential care providers.
* Obtaining services (residential and non-residential) as indicated in a client’s support plan to meet their needs for social care services following an assessment.
What we are looking for:
* Good communication and IT skills.
* Experience of accurate data entry, analysis and reporting from spreadsheets and databases.
* Ability to work well within a team.
