Advanced Clinical Practitioner Nurse/Paramedic
North Care Network is seeking an enthusiastic, motivated, and committed Advanced Clinical Practitioner (Nurse or Paramedic) to join our vibrant and welcoming PCN. A Post Graduate qualification in Autonomous Practice and a non-medical prescribing qualification are essential. Main Duties of the Job
To perform as an autonomous practitioner working independently across our PCN and with other health care professionals. Managing daily surgeries of face-to-face, online, and telephone consultations to assess, diagnose, and treat patients with a range of acute, non-acute, and chronic medical conditions, initiating direct referrals as appropriate.

Job Responsibilities
Clinical Responsibilities:
* Work autonomously and be accountable for own professional actions.
* Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.
* Assess undifferentiated patients with a range of acute, non-acute, and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses, and carry through management treatment plans ensuring relevant follow-up.
* Discuss with registered GP and, where appropriate, make referrals to other health professionals in primary and secondary care.
* Arrange hospital admission for appropriate cases.
* Undertake telephone triage work, telephone advice calls, and online consultations.
* Manage and undertake home visits, including Care Home visits.
* Manage Palliative Care patients.
* Prescribe medication as necessary as a non-medical prescriber.
* Maintain accurate record keeping in SystmOne/EMIS using templates and READ codes.
* Undertake regular medication reviews and medicines management.
* Request and interpret tests appropriate to patients' needs.
* Manage communications from hospitals and other providers e.g. Immediate Discharge Summaries acting on requests for further action, medication, etc.
Person Specification
Experience
* Experience working as an ACP.
* Autonomous working.
Qualifications
* Post Graduate diploma or degree in Advanced Clinical Practice.
* Independent non-medical prescriber.
