Our work in Primary Care Networks (PCN) is at the very heart of Stockport PCN Care. Services will be offered seven days a week to help people remain healthy and build independence. The aim being that the "right care is delivered to the right people at the right time". We are looking for a flexible, innovative practitioner with a variety of clinical skills.
The District Nursing Service provides care in patients' homes as well as residential homes caring for patients with complex needs and supporting carers. Due to the nature of the role, it is essential that applicants are able to travel independently across Stockport and have access to a suitable vehicle for business purposes. Where possible, adjustments can be considered in accordance with the Equality Act.
Stockport PCN is in the position to offer exciting new opportunities for Band 3 Support Workers. We are looking for enthusiastic individuals who would work with the Community Nurses and Adults Social care staff to provide a first-class service to the residents of Stockport. Line management will be provided by Stockport Foundation Trust, under NHS terms and conditions. On appointment, successful candidates will be expected to complete the Care Certificate.
Responsibilities include:
1. Working within a multidisciplinary team delivering social and nursing care to clients in their own home.
2. Care delegated by an appropriate practitioner utilising personalised care plans as appropriate.
3. Providing patients with support in administering medications, eye drops, wound care, and supporting your colleagues.
4. Working 18.00 - 23.00 to support the Evening District Nursing Service.
