To be responsible for the assessment, treatment, planning, delivery and evaluation of evidence-based highly specialised Occupational Therapy/ Physiotherapy intervention within the area of integrated care/Admission Avoidance.
To undertake specialist assessment of patients within urgent care units with acute/unstable conditions who are referred to the Service as an alternative to hospital admission from urgent care.
Main duties of the job
We have an exciting opportunity for a Specialist Occupational Therapist to join our established Home First therapy service. The purpose of the Home First team is to prevent avoidable hospital admissions by supporting patients home from our urgent care footprint of the hospital. This comprises of Urgent Care areas including Emergency Department, and assessment areas. We work in close collaboration with the Frailty Team and community services to deflect admission. The successful candidate will be working within the multi-disciplinary and multi-professional team and will be a key player in providing specialist assessment, personalised care planning and management of care to patients who may require support planning to enable them to remain at home safely. Post holders will take a lead role in mentoring and supporting all disciplines of staff within the team. With full support of managers, you will be instrumental in developing and embedding a culture of seamless working across secondary and community services. The service operates 365 days a year and operates a 7 day rota.

Job responsibilities
* To support the team lead in the day to day management and development of the service and staff.
* To be responsible for Occupational Therapy/Physiotherapy portfolio which will include provision of expert advice to occupational therapists/ physiotherapists, senior clinicians, MDT across Intermediate Tier Services to support them in taking up their duty of care to develop, co-ordinate and deliver training programmes including competency frameworks, information packages for patients and families together with colleagues working in health, education and social care.
* Management of a clinical caseload of patients with highly complex and/or additional Occupational Therapy/ Physiotherapy needs.
* Provide clinical triage and support service demand management through in-reach to the Hospital urgent care units.
* To establish individualised management and treatment plans, using advanced clinical reasoning and utilising a wide range of evidence based treatment skills and options to formulate highly specialised interventions and programmes of care, which will include working with staff across organisational boundaries.
* Provide clinical leadership and supervision facilitating a culture of learning and reflective practice within Occupational Therapy/ Physiotherapy.
* Identify audit/research opportunities, initiating, designing and implementing agreed projects as required.
* To provide mentorship and supervision to junior/ student occupational/ physiotherapy students and assistant staff.
