Job summary Adderley Green Medical Services is looking to recruit an experienced, enthusiastic and highly motivated General Practice Nurse to join our friendly nursing team. Main duties of the job The post holder must be experienced & competent in all areas of general practice nursing and you will Assess, plan, develop, implement, and evaluate programmes to promote health and wellbeing and prevent adverse effects on health and well being Deliver and manage long term condition clinics Delivery of frail elderly assessments Implement and evaluate individual care plans for patients with a known long-term conditions Identify and manage as appropriate treatment plans for patients at risk of developing a long-term conditions Deliver opportunistic health promotion & lifestyle, NHS Health Checks Work with patients in order to support adherence to prescribed treatments Assess and care for patients presenting with minor injuries Support and advise in relation to family planning and contraceptive needs Deliver cervical cytology screening clinics Deliver vaccination and immunisation programmes for both adults and children Home visits to the houseboundfor frail elderly, long term condition reviews, and delivery of vaccination programmes. Administer vaccinations for patients travelling abroad Promote use of PGDs Participate and lead on QOF & QIF- local quality frameworks & enhanced services.
