Job Overview
This is an exciting opportunity to be part of one of our five excellent supported living services in Wandsworth. You will be supporting tenants with a range of needs, including learning disabilities, physical disabilities, behaviour which may challenge, and mental health needs. You will help service users maximize their independence and increase community connections.
Responsibilities
1. Plan and carry out transition work in collaboration with service users, their families, and wider support networks.
2. Provide direct care and support as a Key Worker to help people achieve their dreams and goals.
3. Work flexibly across the week, including evenings, weekends, and bank holidays.
4. Engage service users in planning their support.
5. Communicate professionally with Care Managers, health professionals, families, carers, and friends.
Qualifications
While relevant experience is valued, it is not necessary if you can demonstrate a ‘can-do approach’, person-centred values, energy, and commitment. You will need:
1. Good verbal and written communication skills.
2. The ability to develop high-quality support plans.
3. A strong work ethic and duty of care.
4. To contribute to a positive, team approach.
Training and Development
If you do not already have a relevant vocational qualification in health or social care, we will support you to complete the Care Certificate and an accredited Health & Social Care Diploma.
