Hours: As and when required (to cover sickness and annual leave)
Salary: £12.74 with enhanced weekend rate of £13.85
Location: St Richard’s Hospital, Chichester & Bognor War Memorial

Are you passionate about helping vulnerable individuals return home safely from the hospital? Age UK West Sussex, Brighton & Hove (AUKWSBH) is seeking a compassionate and proactive Bank Take Home & Settle Coordinator to support older individuals by providing a safe discharge home and ensuring their wellbeing once back at home.
About the Role
As a Bank Take Home & Settle Coordinator, you will:
* Drive individuals home in your own car, ensuring they are comfortable and prepared.
* Build relationships with hospital staff to promote the service and manage referrals.
* Provide practical support at home and refer clients to ongoing services where needed. Support the Senior Coordinator in delivering and developing this vital service.
This flexible bank role covers sickness and annual leave across St Richard’s Hospital (Chichester) and Bognor War Memorial. Shifts are typically 11am - 5pm on weekdays and 11am - 1pm at weekends on an ad-hoc basis.
Key Responsibilities
* Promote the service within the hospital to meet contracted targets.
* Take referrals, ensuring discharges are safe and appropriate.
* Assist individuals by:
o Confirming it’s safe for them to return home.
o Obtaining discharge sign-off from hospital teams.
o Ensuring they are happy and prepared for the journey home.
o Providing practical support (e.g., preparing drinks, snacks, and essential food items).
o Positioning hospital equipment and conducting safety checks (with consent).
o Ensuring their home is warm and suitable.
* Refer clients to ongoing support services as appropriate.
What We’re Looking For
Essential:
* Strong communication and interpersonal skills.
* Experience with older or vulnerable individuals, ideally in a community setting.
* Proficiency in administration and IT (Microsoft Office).
* Ability to work independently and as part of a team.
* Knowledge of information governance, consent, and adult social care.
* Full driving license and access to a vehicle for client transport.
Desirable:
* Familiarity with the voluntary sector and inter-agency collaboration.
* Experience using project frameworks and systems.
