Arrange appointments for patients on the PCNs case load, or otherwise, as directed by clinicians, following identification or urgent and non-urgent clinical need, to assess diagnose and treat according to the patients health needs. This includes being responsible for the co-ordination of pharmacy services clinics, which will require reports running, creation of clinics, appointment booking, arranging follow up appointments at suitable times, ordering blood tests, sending and looking at patient feedback and updating figures from patient system. Running reports from our clinical systems is required regularly and ad hoc to monitor for safety and effectiveness of PCN services involving medicines .Working with the Lead PCN Pharmacist to ensure the reports are reviewed and actions taken forward. Provide support and administration to the clinicians carrying out care home medication reviews including liaising with the care homes to gather necessary information and booking appointments ensuring communication is maintained at all times. Provide administration support and co-ordination twice yearly for the COVID-19 and Flu vaccination campaigns, particularly in the delivery of vaccinations to care home residents.. Responsible for keeping the electronic filing system up to date and in an orderly system. This includes ensuring all pharmacy SOPs and policies are up to date. Post holders will need to demonstrate flexibility and adaptability to working in a dynamic and busy environment. Refer patients through to the appropriate team member, and/or make referrals on behalf of the team. Signpost patients to appropriate community or local services. Serve as the contact point, for practice staff, providers, care teams, family/caregivers and community resources where indicated, responding with empathy and respect. Support the PCN to deliver on quality metrics, such as QOF, KPIs and locally commissioned enhanced services. Supporting GP practices in meeting the medicines management incentives set by the Intergrated Care Board (ICB), with progress tracking, signposting and administration support. Post holders will need to demonstrate flexibility and adaptability to working in a dynamic and busy environment. Provide vital signs monitoring (after appropriate training). Ad hoc administration duties such as typing, scanning, meeting room bookings, ordering team equipment.
