The role in the main will be from 8AM to 4PM but there will be some flexibility that will be needed to complete the role effectively. The same applies to the working days, whilst it is mostly Monday to Friday, this may not always be the case as the primary role is to assess people who are medically fit for discharge to be admitted in to one of our care homes nearby.

Job Purpose:
The role of a Trusted Assessor is pivotal in reducing delayed transfers of care from acute settings. Working as part of a team, and in close collaboration with multidisciplinary discharge teams and CHD Care Homes/Care at Home, the Trusted Assessor will complete an assessment of need on hospital patients deemed fit for discharge into a care home/community setting.
The main responsibilities of this post will be:
• Ensuring patient assessments are completed in a 2 to 4 hour time window and their onward destination
arranged and agreed between all relevant parties;
• Negotiating between care home managers and service users to facilitate discharge effectively to the
agreed destination within 24- 48 hours;
• Partnership working with health and social care colleagues to maintain effective patient flow through
the health and social care system;
• Ongoing relationship development with hospital discharge teams and the respective care setting
management teams to raise the profile of the Trusted Assessor Role;
• Development of the Trusted Assessor pathway and according processes;
Responsibilities:
1. To undertake assessments and reassessments on patients (who will experience a variety of health conditions) on behalf of CHD’s care homes/Care at Home, thereby facilitating a safe and timely discharge from hospital ensuring the persons requirements are within the agreed specification.
