We are seeking a skilled and compassionate Nurseto join our innovativeVirtual Wardteam. In this role, you will be responsible for delivering and coordinating high-quality, patient-centred care for individuals receiving treatment at home, supporting their transition from hospital care and preventing unnecessary admissions.
As aBand 6 Nursein the Virtual Ward, you will play a key role in managing the day-to-day in-person care of patients with complex health needs, including respiratory, cardiac, and other long-term conditions. Working closely with consultants, GPs, and other healthcare professionals, you will provide clinical expertise and ensure the smooth delivery of patient care plans.

You are an enthusiastic and experienced nurse with strong clinical skills and the ability to work both autonomously and as part of a team. You are passionate about patient-centred care and excited by the opportunity to work in a pioneering Virtual Ward environment.
Key Responsibilities:
• Provide high-quality clinical care to patients on the Virtual Ward, managing their treatment plans and ensuring patient safety.
• Perform clinical assessments, using observations and point-of-care testing to track patient progress and identify early signs of deterioration.
• Daily management of patient care, ensuring timely interventions and escalation where necessary.
• Coordinate and collaborate with the multidisciplinary team, including consultants, GPs, AHPs, and community services, to provide seamless care for patients.
• Provide guidance and support to junior nursing staff, helping to develop their skills and ensure adherence to best practices in patient care.
• Contribute to the delivery and review of individualised care plans, adjusting as necessary to ensure optimal patient outcomes.
• Assist with discharge planning and continuity of care, ensuring patients safely transition from hospital to home.
• Promote patient empowerment by providing education and support to help patients and their families manage their conditions at home.
• Ensure compliance with clinical governance, infection control, and safety protocols
