Key Responsibilities:
* Provide advanced clinical care, assessment, and treatment to a diverse patient group in a variety of healthcare settings.
* Utilize your expertise to support the clinical team, providing mentorship, guidance, and training to junior staff.
* Act as a clinical leader within the team, collaborating with multidisciplinary professionals to ensure the highest standards of patient care.
* Conduct comprehensive assessments and develop personalized care plans for patients, with a focus on evidence-based practice.
* Manage and review patient progress, ensuring ongoing care delivery is of the highest standard.
* Participate in audits, service evaluations, and clinical governance activities to continuously improve service delivery.
* Maintain up-to-date knowledge of current clinical practices, guidelines, and regulations.
* Develop and maintain strong relationships with patients, caregivers, and other healthcare professionals.
Essential Requirements:
* GPHC Registration as a pharmacist with post-registration clinical experience.
