Our client is seeking a dedicated Assistant Practitioner to join their Health & Social Care department, providing exceptional care and support to service users and their families. Working under the direction of registered professionals, you will play a key role in delivering high-quality, person-centred care.

Key Responsibilities:
1. Provide outstanding care and support to service users under the guidance of registered professionals.
2. Collaborate within a multidisciplinary team to deliver person-centred care.
3. Contribute to the assessment, planning, and evaluation of care plans.
4. Support junior team members and Trainee Assistant Practitioners.
5. Maintain service user confidentiality and uphold professional standards.
6. Promote choice, dignity, and respect in all aspects of care.
Skills and Experience Required:
1. Level 5 Foundation Degree in Assistant Practitioner (Healthcare)
2. Experience working in healthcare, with a preference for those with neuro or palliative care experience.
3. Understanding of relevant legislation and its application in practice.
4. Effective communication and interpersonal skills.
5. Ability to work independently within the scope of practice.
