Job overview
We are inviting you to join our team as a band 6 community rehabilitation Physiotherapist here in Trafford Local Care Organisation. You will be working within a multi-disciplinary team consisting of physiotherapists, occupational therapists and patient support worker colleagues. You will be responsible for a varied caseload delivering evidenced based rehabilitation programmes within patient’s homes and outpatient facilities.
The community rehabilitation service is supported by the AHP leadership model within Trafford. You will work closely across the organisation attending band 6 peer groups where we support shared learning. You will be supervised by band 7 senior practitioners and will have the opportunity to supervise rotational band 5 staff, support workers, and students.
You must have access to a car and a UK driving license, and be prepared to travel across the borough.
Main duties of the job
You would need to have specialist assessment and treatment skills, be able to manage a complex caseload autonomously and be able to work within a multidisciplinary team completing joint assessments, treatment and goal setting as required. You need to have experience in all aspects of rehabilitation with adults, including MSK, respiratory, neurology and orthopaedics.
* Undertake specialist physiotherapy assessment of patients and provide treatments specific to the patients needs and goals.
* Work within a multidisciplinary team, completing liaison, joint working and collaborative decision making.
* Demonstrate a high level of manual handling skills.
* Assess and manage clinical risk and safeguarding, following trust policies and procedures.
* Support and deputise for the Senior Practitioner in the day to day running of the team.
* Contribute to service development.
* Supervise, monitor and delegate to staff including band 5 Physiotherapists, students and support staff.
* Maintain own continuing professional development and develop current knowledge of evidenced based practice.

Qualifications
* Physiotherapy Degree
* HCPC registration
* Coaching qualification/motivational interviewing
Travel
* UK driving licence and access to a car or means of transport
Experience
* Relevant experience within the NHS
* Experience of rehabilitation with adults
* Experience working with community services in the NHS
* Supervision of junior staff/students
* Experience working in an MDT
* Wide range of clinical experience in MSK, respiratory, neurology
Skills
* Advanced moving and handling skills
* Ability to complete thorough Physiotherapy assessments
* Formulate patient specific treatment plans for patients with complex needs
* Complete goal setting in collaboration with patients
You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
