Job summary Dene and Stour Valleys PCN seek one enthusiastic and proactive Care Coordinators to join their team. Dene and Stour Valleys is a patient focused forward thinking PCN looking for people to work with the PCN team to deliver care to their patient populations. The Care Coordinator role is a new and evolving role within NHS Primary Care Networks. The post holder will require a flexible approach to work, the role will consist of some focussed project work as well as general core support to the PCN.The post holder would ideally have good knowledge of community or primary care sector and strong IT skills as the role will be key to practice searches and audits. Main duties of the job To summarise and read code relevant information from patients medical records onto the practice clinical IT system (EMIS), working in accordance with the practice notes summarising protocol. Invite newly registered patients when necessary for overdue screening/immunisations. To prepare medical records/extracts for SARS requests, completing insurance reports, deal with patients queries relating to medical records, completing GP record requests to other GP practices, solicitors etc, sending securely as per Practice protocol. Proficient IT skills (including experience of Microsoft office) is also required as the post holder will also be undertaking a variety of administrative duties to assist in the smooth day to day running of the practice, including some data inputting and contacting patients to encourage them to attend for their national screening invites, e.g. smears, breast screening, bowel screening etc. Through correspondence and telephone queries, deal with patients, relatives, carers, GP's etc, promptly and efficiently exercising own judgement to resolve patient queries within their professional constraints.
