Job summary The Somerset NHS Foundation Trust is seeking a dedicated Clinical Team Leader for our Psychiatric Liaison Service, which plays a crucial role in supporting the mental health needs of patients treated for primary physical health conditions. Liaison psychiatry services are essential, as individuals with physical health conditions experience mental health issues at rates two to three times higher than the general population. In hospital settings, this rate is particularly pronounced, with approximately half of all inpatients affected by conditions like depression, dementia, or delirium. Our Psychiatric Liaison Service provides critical assessment, intervention, and consultation for patients experiencing acute psychiatric episodes, including psychosis, severe depression, dementia, and delirium, as they present in acute hospital settings. The service aims to ensure that patients' mental health needs are integrated into their overall care plans, promoting both mental and physical health outcomes. Main duties of the job To provide a clinical and managerial leadership role for the Psychiatric Liaison Team based at Yeovil District Hospital and be a member of the operational team influencing and promoting improved, dynamic and effective clinical practice across the service, which also covers Yeovil District Hospital. Liaison Psychiatry bridges the gap in general hospitals between physical and mental health to ideally meet the needs of the 30-60% of all patients who may be experiencing mental health illness and comorbid physical health conditions as inpatients and presenting to the ED. The services seek to provide parity of care and esteem for those experiencing mental health illness whilst in hospital, and in doing so act as a conduit for upskilling and educating the wider hospital staff and services. The service focuses on assessment, admission avoidance and discharge planning from ED.
