Are you ready for the next chapter in your ACP journey? Wiltshire Health and Care have an exciting part-time 0.8 wte (30hrs) opportunity for a fully qualified or trainee ACP to join the inpatient team at Wiltshire Health and Care. You will be based on Cedar Ward, Chippenham Community Hospital working alongside a clinical team comprising Consultant Practitioners/Associate Specialists/GP trainees and other ACPs supporting the ward teams with the medical aspects of patient care.
Clinically working independently at an advanced clinical level in the direct delivery of high-quality care making critical clinical decisions based on advanced clinical skills assessment. Provide clinical leadership and support within both the medical and nursing teams and to support the Senior ACP Leadership Team in developing ACP service to improve the patient experience. Support Junior colleagues to achieve training and development requirements. Ensure that you are maintaining your own portfolio of learning in accordance with trust ACP policy and HEE guidance. Ensure clinical areas with consistent high-quality care through audit and research programs.
Applications are invited from candidates who are a Registered Nurse or Health Care Professional, holding a MSc in Advanced Practice (or working towards) and have relevant post-registration experience. Experience at Band 6 or above with knowledge of effective change management skills and a proven track record in the delivery of high-quality clinical care and Non-Medical Prescribing are essential. We are looking for an enthusiastic and dynamic individual with clinical expertise and a passion for care of the elderly within a community rehab setting. The successful candidate will be a team player who is flexible and adaptable to the needs of the service, who enjoys working in a multidisciplinary team delivering high-quality care.
Essential Qualifications:
* Registered Nurse, current registration with the NMC
* MSc in Advanced Clinical Practice or Advanced Nurse Practitioner
* Relevant postgraduate Community Nursing/Acute Nursing specialty qualification/training
* Registered Independent Non-Medical Prescriber
* Leadership qualification/training
Other Requirements:
* A clean driving licence, access to a vehicle for daily use.
