Walkden Medical Centre provides a busy and varied patient-focused environment with a genuine team approach to patient care, consisting of 6 GPs, 2 ANPs, 3 Practice Nurses, Healthcare Assistant & Phlebotomist.
We are looking for a highly experienced Practice Nurse to join our team. The candidate would be someone who is motivated and committed to providing high-quality care. They will work collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.

Main duties of the job
Ideally the candidate has worked in a GP Surgery, should have a strong emphasis on good teamwork in addition to sound decision making and excellent interpersonal skills and have the ability to manage own workload.
Duties will include:
* Provide care for the presenting patient, long-term condition management, cervical cytology, travel & child immunisations, ECGs, routine injections, family planning, home visits, health promotion, support team with clinical emergencies.
Experience of using EMIS Web would be advantageous.

Job responsibilities
Job Summary:
The post holder is an experienced nurse, who acting within their professional boundaries will provide care for the presenting patient from initial history taking, clinical assessment, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

* Registered General Nurse (Registered with the Nursing & Midwifery Council).

Experience
* Recent and relevant experience of working in Practice Nurse role within Primary Care.
* Recent and relevant experience of long-term condition management.
* Maintenance of up-to-date skills, knowledge and ongoing competence of the Practice Nurse role.
* Evidence of capacity to work successfully, both as an autonomous clinician and as part of a team.
* Proven ability to evaluate the safety and effectiveness of own practice.
* Interpreting and implementing local and national policies.
Knowledge
* Understanding and knowledge of policy developments related to the delivery of primary care services.
* Understanding of systems to gain an understanding of the health needs of the Practice population.
* Understanding evidence-based practice.
* Knowledge of national standards that inform practice (e.g., National Service Frameworks, NICE guidelines etc.)
* Accountability of own role in line with NMC Code of Professional Conduct (2015) and medico-legal aspects of the Practice Nurse role.
