Providing effective clinical leadership to ensure all nursing staff receive the appropriate supervision and support in conjunction with an agreed professional development plan.
Establishing and maintaining team systems and processes for effective operation of the team, including outcome measures and key performance indicators.
Main duties of the job
1. Ensure effective nursing assessment and provision of a high standard of care within the community specialist learning disability service.
2. Act as a positive clinical lead and role model within the community specialist learning disability service.
3. Set and monitor care standards for the community nursing team to help support the provision of high-quality care.
4. Facilitate others in the team to develop competence by providing leadership, day to day supervision, clinical advice and clinical supervision.
5. Advocate for and promote a positive image of people with a learning disability.
6. Provide advice on reasonable adjustments to primary and secondary healthcare services.

Job description
Job responsibilities
1. Ensure a person-centred approach providing a high standard of service user care that is in accordance with relevant legislation, such as the Mental Capacity Act.
2. Provide specialist clinical advice and undertake clinical risk assessment and planning.
3. Promote and participate in multi-disciplinary assessment and treatment planning.
4. Work with service users and their families/advocates to offer clinical insight based on best practice in the formulation of person-centred health and wellbeing assessments based on individual choice and need.
5. Ensure that care providers are aware of the theoretical and practical reasons for the clinical interventions for each service user and take a lead role in the delivery of relevant training.
6. Identify and provide training in response to specific service user needs.
7. Ensure all nursing staff are updated on all aspects of medicines management.
8. Ensure that service user records and documentations are kept in accordance with Trust and NMC policies and guidelines.
9. Act as a role model in the delivery of care.
10. Promote learning opportunities for staff working in the service area.
