An exciting job role has recently become available at the Southwark. They are looking for a new member for Transfer of Care.
What will your responsibilities be?
As an Adult Social Worker in the Transfer of Care team at Southwark, you will conduct comprehensive assessments of adults’ needs, risks, and strengths to facilitate safe and effective transfers of care. You will develop and implement person-centred care plans, collaborate with health, housing, and other agencies to ensure seamless transitions, and provide crisis intervention. Your role includes safeguarding, managing a caseload of individuals with complex needs, and empowering clients to advocate for themselves. You will ensure compliance with statutory requirements and engage in continuous professional development to maintain and enhance your skills and knowledge. These responsibilities aim to provide high-quality, timely, and sustainable social work services to adults undergoing transfers of care.

Qualifications and Experiences :
You must have Social Work England registration, be eligible to work in the UK, and hold a full UK driving licence. Post-qualified experience and a degree level or equivalent qualification in Social Work are also required.
