Leadership of Services Delivery
You will support the delivery of services that provide the best life possible for people with learning disabilities with your support team:
* Provide lead practitioner expertise and lead the support worker team on a day-to-day basis for the designated services or activities, ensuring high standards of care and support are always maintained and that the designated services continuously improve and develop.
* Deliver and demonstrate best outcomes for all individuals supported by Almond Care and ensure their involvement in shaping these using the principles of co-production.
* Work with your team to ensure that the individuals we support have accurate, detailed, and person-centred support plans, one-page profiles, and person-centred reviews.
* On behalf of the Head of Supported Living, lead on ensuring a flexible and person-centred rota is always in place for the service that meets the needs and wishes of the individuals we support, ensuring that the support team has adequate notice of their roster pattern.
* Under the direction of the Head of Supported Living, coordinate all matters relating to medication, ensuring all procedures are rigorously followed and all support staff are appropriately trained.
* Conduct supervisions and performance appraisals with your team as designated, in line with organisational policy, completing individual learning and development plans which address training needs and support continued professional development in conjunction with the Learning and Development team.
* Work in partnership with social workers, families, advocates, and other providers to ensure that support is provided collaboratively with the individual at the centre.
