Job summary We have an exciting opportunity for an enthusiastic and self-motivated Physiotherapist, to join our community inpatient therapy team at York and Scarborough Teaching Hospitals NHS Foundation Trust, based at Nelsons Court. The post will offer the opportunity to develop clinical skills in the management of patients within our community inpatient rehab services. This will be exclusive to older adults with a range of clinical conditions, so an exciting opportunity to broaden your knowledge and experience Alongside this there would be a requirement for the candidate to contribute to service and professional development, support student practice placements where appropriate and supervise colleagues within the services. There will also be an expectation to work across our 3 community inpatient units as service requirements arise. The ability to transport yourself and equipment across the locality during the course of a normal working day is required for this vacancy. Access to a vehicle that can be used for business purposes and hold a full and valid driving licence (role will involve travel to numerous addresses that cannot be easily accessed by public transport means) is required. Main duties of the job To undertake comprehensive assessment of patients working autonomously within professional boundaries and with supervision available from senior Physiotherapists To formulate and deliver an individual Physiotherapy treatment plan based on a sound knowledge of evidence-based practice, treatment options, and clinical reasoning and assessment skills. To deliver appropriate manual physiotherapy techniques within the patients' management plan, this will involve high levels of dexterity, co-ordination and accuracy To ensure a holistic approach to the overall care and rehabilitation including effective working relationships with MDT, wheelchair and specialist equipment providers, orthotists consultants and relevant others. To ensure sensitive and appropriate handling of information in accordance with the data protection act and Trust policy. To provide training and advice on Physiotherapy interventions, activity, injury prevention, changes and adaptations to the patient's lifestyle considering the environmental, emotional, physical, spiritual and social needs. To address functional physical, pain and skills deficits enabling the patient in areas of self management, health and wellbeing, To be responsible for organising and planning own caseload, to meet service and patient priorities. Readjusting plans as situations change/arise.
