Job Title: Qualified Social Work Community Hospitals Salary: £32,654 - £40,476 per annum plus a £3,000 market forces payment 37 Hours per Week Location: Newhaven Ward, Princess of Wales Community Hospital Stourport Road Bromsgrove B61 OBB Team: Urgent Care / Community Hospitals Permanent Contract The Role: The Community Hospitals Team have an exciting opportunity for an Experienced Social Worker. We are keen to hear from you if you enjoy working in Multi -Agency Setting but have strong Social Worker ethics and values. On this occasion we will not be accepting applications from Newly Qualified Social Workers. We are a large supportive team supporting patients in the Community Hospitals, those in Discharge to Assess Beds in Nursing Homes and the community Wrap Around Service. We work together with our Colleagues in the Acute Hospitals both in and out of County and our Health Colleagues both on the ward and in the Community. This post is based at New Haven Unit where there are two wards, Woodland and Meadow, which provides specialist care to Older Adults that are experiencing difficulties with their mental Health. If you are the successful candidate you will be working primarily with those adults who have a Dementia or a Dementia type diagnosis. You will be working with some many patients who have Complex Care Needs and as such a working knowledge and significant experience of Continuing Health Care is key as is Care Act Eligibility. Some of those on the ward will have been detained under sections of the Mental Health Act so although you won't be completing Mental Health Act Assessments you will be required to have an understanding of this work. You will be working with a number of patients who are cognitively challenged so you will also need to demonstrate you have experience of completing Assessment under the Mental Capacity Act. We would also expect you to have sound understanding of the Mental Capacity Principles and adhere to these when working on the wards. Safeguarding and working positively with risk is part of role as well as understanding the need to support Carers and Families at what can be a stressful overwhelming time. While we are keen to minimise unnecessary road journeys we do at times have referrals from our colleagues in Hereford and would expect you to be able to travel there to carry out assessments if required. The Team meets regularly in Team Meetings where we have guest speakers and in smaller cluster meetings with ASWP’s where workers have an opportunity to share aspects of good practice or share some of the challenges. You will have regular Supervision and peer support to ensure you are able to continue your career development and career aspirations.
