Do you want to be part of shaping a new project? We are seeking a Social Worker to join our community outreach support service. If you believe this could be you then don’t hesitate to get in touch You will be highly motivated, with lots of initiative, and will demonstrate the ability to adapt your interventions in response to all kinds of people, in a range of working environments; and be naturally collaborative, bringing to the role a flair for building relationships, and working in partnership. You will have the ability to assess where families are struggling and intervene in a strengths based manner to help the family feel supported and gain resilience. We will provide structured levels of support, which are flexible and centred around the individual. We work together with our service users, their families and wider support networks to build independence and a positive sense of self. You will work to maintain active involvement within the local area, enabling people to access the community around them. Your role will include: Working with families in crisis. Assessing the needs of service users and carers, you will plan, implement, review, and evaluate outcomes; working for and with individuals, taking care to understand their unique circumstances and priorities. Developing trusting relationships with service users Maintaining positives links with community organisations and services Devising and reviewing outcome focused support plans Assessment and management of risk Encouraging service users to develop self-care and independent living skills A current driving licence is an essential requirement of this post.
