Job summary
Within our busy Neighbourhood Team, we arelooking for a Community Nurse hours per week, who is passionate about nursingand excited by change and who is keen to help shape a new way of deliveringnursing care in the community. We are ateam of creative health care professionals who are focused on compassionatecare. We work closely with our GPs and work hard at developing relationshipswith other primary care services, drawing on the skills of our MDT colleaguesto manage complex patients and ensure our services are as responsive andpatient centred as possible.
We are a supportive team who are allcommitted to developing our services to provide the most individual care forour patients. We offer dedicated time for training and clinical supervision,with a supportive learning & development department to help guide you inyour career progression.

Main duties of the job
The role requires

Your typical working day willinclude home visits for adults over 18 years needing a range of nursinginterventions, including end of life care, all types of wound care- you willbecome expert in tissue viability. Other skills required will be in IV therapy andholistic assessment of a persons needs, taking in to account their environmentand social circumstances.
Youwill support in the education of student nurses pre and post registration, plusnurse associates and HCAs to a high standard. As a band 5 nurse, you willoversee patients and assist in decision making regarding their care. You willalso attend daily handovers and participate in meetings.

The post holder will:
