Main duties of the job
NHS Sussex ICB is in the process of positively developing its Childrens Continuing Care (CYPCC) services and expanding our team. We are recruiting a Registered childrens nurse or allied professional to join our existing Childrens continuing care team, to work across Sussex as a Pan Sussex CYPCC team assessor.
This role as assessor requires acompassionate approach,within a Multidisciplinary team, to establisheligibility using the NHS National framework for children and young personscontinuing care, and liaising with Health, Social care, Education colleagues andother professional colleagues and agencies, to oversee, with band 7 support,the delivery of child specific packages of care. Line management and supportwill be given from the Band 7 Nurse Specialists within the team, and from the PanSussex Clinical Operational Lead nurse. Some experience of complex, long-termhealth conditions and working in the community with children and young peopleis desirable.
Communication and IT skills areessential to this role. The job is home based with some visits to families homes,schools and respite or residential settings across Sussex will be required.

Job responsibilities
This post is situated within the NHS Sussex Team.Staff will support each other in service delivery across Sussex for allchildren and young people registered with a Sussex GP. This will require thepost holder to work across the Sussex wide county in order to meet the needsof the service.
The Nurse Assessor is responsible for:
Supporting the commissioning of health carepackages at home for CYPCC eligible children and other community settings toprevent hospital admissions; in addition, they will help support earlydischarge from hospital where ever possible, working with the Well Childnurse for Pan Sussex.
In the absence of Nurse Specialists, act in adeputising role, which would include undertaking duty management andresponding to package breakdowns, enquiries for new referrals, checklists andprioritising workload.
To help support the daily duty system for CYPCCacross Pan Sussex, working with the duty nurses; responding in a timelymanner to support the Pan Sussex caseload.
Acting as a member of the multidisciplinary team toco-ordinate the process, establish eligibility and assist in identifying asuitable care package or respite setting. The post holder will be responsibleto ensure that the review process for all commissioned care is undertaken inaccordance with current guidance.
Assessing childrenand young people in the community/ home and hospitals to determineeligibility to receive NHS Childrens Continuing Care. Utilising the nationalassessment tools and adhere to the required timescales. Co-ordinate,participate and support the assessment process with the multidisciplinaryteam (MDT). The role will be part of the MDT and determine the individualseligibility in line with the National Framework for Children and YoungPeople's Continuing Care. This will encompass explaining the process topatients and families, health and social care colleagues and care providers.Ensuring that the process is transparent and clearly understood by allparties. Clinical leads for CYPCC will support throughout this process.
Assist when requiredwith the monitoring and providing assurance on the quality of serviceproviders for Children and Young Peoples Continuing Care, this includesresidential placements and short breaks environments.
Advising CHCProcurement on clinical needs and best practice in relation to services forchildren with complex health needs and providing clinical input to all partsof the commissioning process.
Working alongsidethe NHS Sussex PHB team, assist with the personal health budget (PHB) processfor childrens families / carers who would like to be in receipt of apersonal health budget and ensure that the NHS Sussexs procedures aremonitored alongside the PHB navigator.
To be responsiblefor planning, managing, co-ordinating and undertaking annual reviews of the Children and young peopleseligible for Continuing HealthCare, within the Pan Sussex caseload.Responding to any health needs reported and reviewing the DST and carepackage accordingly.
