Job summary Rotherham Hospice is seeking a compassionate and experienced Clinical Nurse Specialist to join our Hospice at Home Service Team. Reporting to the Head of Hospice at Home the Clinical Nurse Specialist plays a crucial role in delivering high-quality palliative and end-of-life care to patients in their own homes. With a focus on holistic and person-centered care, the Clinical Nurse Specialist provides expert clinical support, assessment, and coordination of care, working collaboratively with patients, families, and multidisciplinary teams to ensure the best possible outcomes. 2 or 3 x 12.5 hours,Rota weekends bank holidays, may need to do nights Main duties of the job Provide expert clinical assessment, monitoring, and management of complex palliative care needs for patients receiving care in their own homes Develop and implement individualised care plans in collaboration with patients, families, and other healthcare professionals, ensuring holistic support and symptom management Offer emotional support, education, and guidance to patients and their families, empowering them to make informed decisions about their care and treatment preferences Act as a key point of contact for patients and families, providing ongoing communication, reassurance, and advocacy throughout their care journey.
